At KentuckyCare, we strive to provide the highest quality of care possible for our patients. The Coordinated Care Team offers services that assist patients with receiving the care needed to improve and promote a higher quality of health. By participating in these services, a comprehensive treatment plan is developed to promote physical, mental, and emotional well-being while also helping you to manage conditions.
Parts of a coordinated care plan might involve some or all of the following services:
- Chronic Care Management
- Diabetes Self-Management Education & Support
- Diabetes Prevention Program
- Medical Nutrition Therapy
- Remote Patient Monitoring
- Continuous Glucose Monitoring
- Intensive Behavioral Therapy
Our providers will determine which services are needed based on several factors, including the severity of your condition, your usual day-to-day activities, and the type/types of medical conditions you have. Patients who come to KentuckyCare can rely on us to help determine the right types of care needed to improve quality of life!
